Home and Community care is provided to Beneficiaries of the Labrador Inuit Land Claim Agreement living in Nunatsiavut, Happy Valley-Goose Bay, Mud Lake and Northwest River. The program is delivered in close collaboration and communication with families and partners such as Labrador Grenfell Health. The program is flexible and adaptable to address the changing needs of clients in the program. The role of the Home and Community Care Program (HCCP) is to support the services provided by the family, the community and other resources.  The HCCP, which consists of the Home Support Services program (HSSP) and Home Care Nursing, is designed to support the health care needs of the client and family in the community. Services provided by the HSSP includes home management, personal care, palliative care and in-home respite. These programs and services include, but are not limited to:

  • Structured client assessment
  • Case management
  • Program management and supervision
  • Maintenance of client records
  • Data collection
  • Access to medical supplies and equipment
  • Home oxygen therapy
  • Chronic disease management
  • Foot care
  • Palliative care

All components of the HCCP are delivered in Nunatsiavut. In Happy Valley-Goose Bay, Mud Lake and Northwest River only the HSSP (home management, personal care, palliative care and in-home respite) is delivered. Home oxygen therapy and foot care is also available for clients in Happy Valley-Goose Bay, Mud Lake and Northwest River who meet admission criteria.
